Therapeutic Services Intake Form

Individualized Therapeutic Services at The Bridge Center empower children and adults to build skills, gain confidence, and reach meaningful goals through evidence‑based, client‑centered care.

Therapeutic Services at The Bridge Center

At The Bridge Center, our licensed therapists provide exceptional Physical Therapy, Occupational Therapy, Speech and Language Pathology, and hippotherapy as part of personalized treatment plans. We work closely with each individual and family to support progress, independence, and participation in daily life.

Insurance Acceptance Update

We are currently in‑network with Mass General Brigham and Blue Cross Blue Shield of Massachusetts, and we are in the final stages of accepting insurance from Harvard Pilgrim. As additional plans become active in our system, we will update our Insurance and Billing at The Bridge Center webpage.

Get Started Today:

  • Step 1: Request an evaluation via our Intake Form below.
  • Step 2: A staff member will contact you to schedule your first visit, verify eligibility, and answer any questions you may have.
  • Step 3: After your call with our staff and once eligibility is confirmed, complete your electronic Intake Forms through the Patient Portal on Jane App.
  • Step 4: Begin individualized therapy, which may include:
    • Physical Therapy (PT): Improving mobility, strength, and independence to help you to move through life with confidence.
    • Occupational Therapy (OT): Building essential life skills, from self‑care to school and workplace readiness, to support full participation in daily activities.
    • Speech and Language Pathology (SLP): Enhancing communication, speech, and language skills to strengthen connection and engagement across environments.
    • Hippotherapy: A treatment strategy in which licensed therapists purposefully use equine movement to achieve functional outcomes.
